About the role
Provide world-class care delivery services to Maven members as a health coach. Coordinate care, drive behavior change, and improve outcomes for higher risk members.
What you’ll do
• Drive program engagement and optimize opt-in rates. • Co-create SMART goals and track member progress. • Manage caseload via messaging and video appointments. • Use motivational interviewing to promote health outcomes. • Collaborate with providers to translate clinical recommendations.
What you’ll bring
• 1-2+ years clinical nursing experience. • Experience with maternity or family planning care. • Training in health coaching and behavior change. • Strong telehealth and digital tool competency. • Ability to communicate empathetically and adapt quickly.
